A technique of closure of refractory macular holes with anterior or posterior lens capsule. 2patients with failure of macular hole to close after primary pars plana vitrectomy ILM peeling &C3F8 gas were assessed with OCT . Resurgery Technique-Case1In a Phakic patient Anterior capsule after rhexis was used to cover the macular hole after fluid air exchange and C3F8 injected. Case2-In Pseudophakic patient posterior capsule was stained with ICG dye and posterior capsulorexhis done using microforceps through pars plana.The posterior capsule then placed on the macular hole & C3F8 injected.Both the cases at 6 weeks follow up had improved vision with closure of macular holes on OCT.Anterior or posterior capsule flap has advantages of being readily available,being more rigid than ILM flap aiding easy manipulation, has higher specific gravity so it easily sinks unlike ILM flap which floats in vitreous cavity.Thus,this is a novel &promising technique for management of refractory macular holes.
