In this video, we try to have a closer look at the reasons of unhappy patients in refractive practice, i.e, dry eyes, haze post PRK (Photo Refractive Keratectomy), pseudo-myopia and flap related complications. Dryness of eyes is the most common side effect of corneal refractive surgery. Better pre-operative evaluation and when documentation of severity of dry eyes is essential. We also look at the molecular biology to look for reason of haze in post PRK and find a clinically relevant suggestion to minimize its occurrence in practice. Pseudo myopia can be one of the causes of refractive surprise, if unrecognised can lead to unhappy patient and unhappy surgeon despite multiple procedures. Flap related complications are rare, but can be difficult to manage when it occurs. We present a case to show how to manage a case of lost flap with the help of topo-guided treatment. Each of the problems are looked in to and a management algorithm is made to for preventing unhappy patients.
