Modern cataract surgery is mostly considered as a refractive procedure where spectacle independence is highly expected .Even in the best hands it’s difficult to have patients within 0.5 D of intended target refraction. With different formulae available, how do we choose which one to use for a specific patient? To shed light on this, we did a retrospective study to evaluate the accuracy of 3rd generation formulae Vs Barrett Universal formula. Patients operated by single surgeon, with pre op corneal astigmatism of < 0.75 D, with insertion of Acrsof IQ IOL over the last 1 year were included. Using optimized lens constants, the predictive refractive outcome using each formula was calculated and compared with actual refractive outcome to give the prediction error. Eyes were sub grouped into short (<22.0mm), medium (22-23.99 mm) and long (>24.00 mm ), the mean absolute error and the percentage of eyes within 0.25, 0.50, 0.75 and 1.00 D were analyzed and the statistical difference discussed.
