Melkersson-Rosenthal syndrome is a rare noncaseating granulomatous disease of unknown cause, characterized by the triad of orofacial edema, facial nerve palsy, and furrowing of the tongue. The complete triad is seen only in 25% of cases. Isolated eyelid swelling is a rare presentation & possibly under-diagnosed.Isolated eyelid edema in MRS is uncommon and poses a diagnostic dilemma. We hereby report a case of 48 yr. old male presented with bilateral periocular , perioral edema ,and unilateral lid edema of varying duration, also having Paralysis of left side of face and granulomatous chelitis.Clinical features and management of this rare syndrome are discussed. The syndrome not always present in the classical triad form but also in the rare periocular form, which may be confused with orbital tumors and orbital pseudotumors. Biopsies should be performed routinely in all patients who present with eyelid edema of unknown etiology. Histological examination is vital for the diagnosis of MRS.
