A 45year old male presented with pain and protrusion of LE since 1 month. RE was normal except for lens changes and in LE axial proptosis with abduction restriction was present. Vision in LE improving to 6/24 and IOP was 16mmHg. Fields and colour vision were normal. Hertels exophthalmometry revealed RE-15mm and LE-22mm with a baseline at 105. Fundus of RE showed a clear media with CDR of 0.8 and in LE CDR of 0.7. LE B-scan showed a well circumscribed lesion behind the globe. MRI of orbit showed, well defined T2 heterointense and T1 isointense extraconal fusiform lesion in the lateral aspect of left orbit and rectus muscle with displacement of left optic nerve and a ring enhancing lesion in left frontal lobe.MRI Chest screening showed left lung upper lobe mass lesion suggesting the primary with metastasis to left lateral rectus muscle and brain. This is a rare case of presentation of unilateral proptosis with primary in left lung upper lobe.
