It is a prospective observational study of patients with endothelial dysfunction who underwent PK or DSAEK. Patients with optic atrophy, macular degeneration or DR were excluded. Routine history, detailed ophthalmic evaluation, cost incurred by patient including cost of medicines and hospital visits was estimated by detailed interview and checking of patient’s medical and expense records at baseline, 6 month, 1 yr and 2 yr after surgery.
Baseline characteristics of the 2 groups with 60 patients each were adjusted for differences. Visual acuity improvement was statistically significant and faster with DSAEK, with lower post-operative spherical equivalent. Both groups had comparable graft clarity and survival. PK continued to remain costlier than DSAEK at 2 years. The ICER of DSAEK over PK was -39052 INR and the ICUR of DSAEK relative to PK was -195260 INR and both were statistically significant at 2 years.
DSAEK is more cost-effective than PK.
