Purpose : To treat and to report a case of large angle exo-deviation in 44/F with 2 steps squint operation.
Methods : A 44/F having 96 PDS Exo-deviation for a long time i.e. for more than 25 years is taken up for squint correction. She was told about the cosmetic correction of squint only but not the physiological function. She is having 6/6P vision in both eyes with alternating exotropia. The left non dominant eye was operated first followed by right dominant eye. 6mm MR and 10mm LR, Resection and Recession was done in both eyes with one week gap. After 6 weeks of surgery her eyes were aligned to the maximum extent (98%) this case was done in a tertiary eye hospital.
Conclusion: Large angle squint deviations can be managed successfully with 2 step squint operation. The residual amount of angle of deviation can be calculated carefully for the 2nd stage operation to obtain maximum success rate.
