Purpose :To demonstrate efficacy of opposite clear corneal incision (OCCI) to treat up to 1 diaptor of against the rule astigmatism and 1.5 D of with the rule astigmatism and Limbal relaxing incisions(LRI) to treat up to 3 diaptors of WTR and against the rule astigmatism during phacoemulsification cataract surgery.

Material and method : 98 Patients with 1D ATR astigmatism and 1.5D WTR astigmatism were selected for OCCI and more than 1d ATR astigmatism were selected for LRI. . Post operative K readings were taken 6 months after surgery. NAPA Nomogram was used to determin the arch of paired incisions for LRI.The incisions were taken with 600 micron diamond blades

Results : 95% of cases up to 1 d achieved correction within 0.5D 80% of cases up to 2D achieved correction within 0.5 D and 75% cases between 2 to 3D achieved astigmatic correction within 1 D

Conclusion : LRI and OCCI are effective method to treat pri existing astigmatism during phaco emulsification cataract surgery.
