The conventional methods of measuring vision relies on assessing visual acuity on standard charts. However this is no longer enough and quality of vision, an important contributor to patient dissatisfaction despite 20/20 acuity, needs to be considered.
It can be affected by the tear film to corneal aberrations, internal optics, pupillary diameter, Nyquist limit and visual pathway terminating into the occipital cortex. The psychological state can also contribute to the optics.
Its assessment is complex and measurement is not yet fully defined. Thus, we attempt to outline the modern metrics of measuring vision as an interplay between multiple components as contrast sensitivity, depth of focus and higher order aberrations, any of which may play the devil in our surgical outcomes.
