Uveitis treatment is asynonymous with steroid bombardment. Steroids being a double-edged sword can backfire if injudiciously used in viral retinitis, tubercular choroiditis, toxoplasma retinitis. Avoiding them altogether can cause so many perils and are the lynchpin in tackling paradoxical reactions. The appropriate titration of inflammation with specific anti-infectives and steroids is the mantra of treatment success. The USP of any uveitis specialist is extending the armamentaria of uveitis treatment from beyond steroids to immunosuppressives with its choices. Where steroids are ineffective in uveitis is an eye-opener to a budding ophthalmologist. Limitations of steroid use with comorbidities without under titration is a tightrope walk. Dynamic assessment of response to steroid treatment is the fulcrum for the astute clinician to clinch several subtle diagnoses.
