A 54 year old man presented with decreased vision both eyes since 3 months. Hoe gave history of using thick glasses since childhood. On examination his vision was 6/18, N8 both eyes with + 10.5Dsph. Anterior segment examination showed shallow AC, VH Grade 1, Nuclear cataract. His IOP was 15mm Hg OD, 17mmHg OS by applanation tonometer. In OD, K1 was 49.13D at 170 ̊, K2 52.98D at 80 ̊. In OS K1 was 48.35D at 7 ̊, K2 49.78 at 97 ̊. IOL master showed axial length of 17.06mm in OD, 17.28mm in OS. IOL power calculated with Hoffer Q formula was 47D for Alcon 60AT in OU. As the power was too high it was decided to do piggy back IOL. Patient underwent phacoemulsification with IOL implantation under local anaesthesia. Before starting the surgery 23G vitrectomy infusion port was placed as precautionary measure. Alcon 60AT +24D was implanted in the bag, Alcon 60BM+22D was implanted in the sulcus. A peripheral iridectomy was done. On day one postop patient had unaided vision of 6/18.
